I think the Brood is nastier than the Fly, although the Fly has some hardcore nastiness. The Brood is pretty damn icky.

Irreversible - without a doubt. close second would be A Ma Soeur (or in the english titled version Fat Girl) - not as horrific as Irreversible but definitely makes you want to take a long shower after.
oh - and murderdoll - you suck- I stupidly clicked on that - now i'm traumatized for life!!!
