Although he's not strictly horror, the Marquis De Sade has written some incredibly disturbing stories.

Clive Barker
HP Lovecraft Edgar Allan Poe James Herbert Anne Rice Ramsey Campbell. Not strictly horror but Iain Banks wrote the scariest book I've ever read-The Wasp Factory and if you don't mind sci fi horror his Iain M.Banks stuff is great too.

The sickest most messed up book I've ever read (or almost read I had to put it down, it was just way to sick) is Hogg: A Novel - Samuel R. Delany. I wouldn't recommend it. Even though the Wasp Factory was disturbing it had a good story, Hogg was just sick to be sick.

Yeah that's why I adore The Wasp Factory-the nasty stuff was backed up with an amazing story. I don't think I would enjoy a book that was just sick to be sick so what you said is duly noted.
