Friday the 13th Part V: A New Beginning ('85) -- It's not really an asylum kind of setting, it's more of a home for the mentally disturbed such as teenage Tommy Jarviss, in the previous film he killed Jason Voorhees with multiple machete blows to the head area and was sent to the home because he was emotionally disturbed.
