I'm gonna go out on a limb here and say that's a shadow. If you notice, she's wearing a pretty tight bra. I think the only way for her boob to fall out of that would be if she were to pull it out on purpose and if she had pulled it out on purpose, I'd be expecting a better picture of it than this. Just my 2 cents. MD can probably confirm or deny whether her titty was hanging out when this pic was taken.
